This shows the interest and tax that apply in a particular tax year. It is usually required in order to complete an Inland Revenue self-assessment form. The form you request will relate to the last completed tax year and will be forwarded to your registered address as held on our records. For information relating to previous years you can use the form at the bottom of your Investment Statement. Alternatively, you can call Customer Services on 0345 850 1722*.
