The following documents cannot be printed out, as they are required in their original format. However, if you would like to request the forms, simply check the appropriate boxes and enter your account number at the bottom of the page. We will then send them to you at the registered postal address for your account.
Forms will only be issued if you currently have an investment account held with the Society. Where more than one copy of a form is required please make a separate request for each investment account.
Section 975 Certificate This shows the interest and tax that apply in a particular tax year. It is usually required in order to complete an Inland Revenue self-assessment form. The form you request will relate to the last completed tax year and will be forwarded to your registered address as held on our records. For information relating to previous years you can use the form at the bottom of your Investment Statement. Alternatively, you can call Customer Services on 0845 850 17 22*.
R85 - Interest without tax deducted The Inland Revenue requires a completed original copy of this form in order to allow interest to be paid without the deduction of tax. Please note that in order for this to apply, the form must be registered when the interest is actually paid, rather than during the period over which the interest was earned.
Investment account withdrawal form Please submit this slip, along with your account passbook (where a passbook has been issued), to withdraw funds from your account. Please read the terms and conditions of your account to ensure that the instructions comply.
